Questions to Ask Before Choosing a Practice

Policies can change, and different doctors within the same practice may approach vaccination differently. Before transferring care, ask directly:

  • Will you continue providing routine and sick care if we decline some vaccines or use a modified schedule?
  • Are there any vaccines patients must receive in order to remain in the practice?
  • Can vaccine decisions be made individually over time?
  • Does the vaccine policy apply to every physician in the practice?

Looking for a Medical Exemption?

Some parents hope that finding a vaccine-friendly doctor will also mean finding a doctor willing to write a medical exemption from school vaccination requirements.

In practice, medical exemptions can be very difficult to obtain — even from doctors who support individualized vaccine decisions.

A medical exemption generally requires an actual medical basis for concluding that a particular vaccine should not be given to a particular child. Depending on the state and circumstances, that may include a documented serious reaction to a previous dose, a recognized contraindication, or another medical condition that makes vaccination medically inappropriate.

The possibility that a vaccine might cause harm is generally not, by itself, a medical exemption.

This distinction surprises many parents. Concern about vaccine risks may be an important reason for a parent to accept, delay, or decline a vaccine, but that is different from having a documented medical basis that a physician is willing to certify as an exemption.

Physicians who sign medical exemptions put their own medical judgment behind them. Many are reluctant to do so and may be concerned about having to defend the medical basis for an exemption if it is reviewed.

A doctor being "vaccine-friendly" does not mean that doctor will write a medical exemption.

If you are seeking a medical exemption, do not assume that finding a vaccine-friendly doctor will solve the problem. Medical-exemption requirements vary by state.
